Terms of Service

1. Acceptance of Terms

By receiving services from Heal At Home Hawaii LLC, you agree to be bound by this Terms of Service Agreement. If you do not agree, you may not receive services.

2. Nature of Services

We provide non-emergency, elective sports medicine services, including but not limited to:

  • Diagnostic Sports Ultrasound

  • Prolotherapy

  • Platelet-Rich Plasma (PRP) Injections

  • Ultrasound-guided musculoskeletal injections

  • Trigger point therapy

All services are cash pay only and not billed to insurance.

3. Payment Terms

  • All fees are due in full at the time of service.

  • We do not accept or submit to insurance, Medicare, Medicaid, or any third-party payers.

  • Accepted payment methods include: credit/debit card; we do not accept cash or written checks

  • A $100.00 deposit will be collected at the time of booking service with the remaining balance collected upon completion of service

  • No refunds will be issued once a service is provided.

4. Scheduling and Cancellations

  • Appointments must be scheduled in advance.

  • Cancellations must be made at least 24 hours in advance.

  • Failure to cancel or reschedule within this window may result in a cancellation fee of $100.00

  • Repeated no-shows may result in denial of future services.

5. Informed Consent and Medical Risk

By receiving services, you acknowledge and agree that:

  • You have had an opportunity to discuss the procedure, potential risks, benefits, and alternatives.

  • No guarantee of outcome or improvement is made.

  • Adverse effects such as bruising, infection, allergic reaction, nerve damage, or pain may occur.

  • You voluntarily assume all associated medical risks.

You will be asked to sign a separate Informed Consent Form for any injection-based procedure.

6. Eligibility and Medical History

  • You affirm that all information provided during your intake, including medical history, is accurate and complete.

  • You agree to notify the Clinic of any changes to your health or medications prior to receiving treatment.

  • We reserve the right to decline or discontinue services based on clinical judgment or safety concerns.

7. Privacy and HIPAA

  • We respect your privacy and comply with all applicable laws, including HIPAA.

  • Your health information will not be shared without your written consent, except as required by law.

  • Please refer to our Notice of Privacy Practices for more information.

8. Limitation of Liability

To the fullest extent permitted by law, you agree to release and hold harmless [Your Business Name] and its providers from any claims, damages, or liabilities arising out of:

  • Your receipt of services;

  • Any adverse outcome;

  • Your failure to follow post-procedure instructions or disclose relevant information.

9. No Provider-Patient Relationship Established via Website

Use of our website, social media, or communication tools does not establish a provider-patient relationship. Medical decisions will only be made during in-person visits or approved consultations.

10. Dispute Resolution

Any disputes or claims arising out of these terms or services provided shall be resolved through binding arbitration in Honolulu, HI, and governed by the laws of the State of Hawaii.

11. Changes to Terms

We reserve the right to modify these Terms of Service at any time. Changes will be effective immediately upon posting. Continued use of services indicates acceptance of the revised terms.
